DHS General Consent Form
Authorize DHS to discuss your case with a navigator. Print, fill out, and send to Health Access MN only if requested by a navigator.
Account Creation Form
Plan and keep track of your username, password, and identity questions. Health Access is not allowed to keep this information on file for you.

How to Manage Your Assister Portal
If you are working with a MNsure navigator, you can give them permission to act on your behalf to submit an application or enroll in a plan. Here are instructions on how to use the “Manage my Assister” feature in your online account to add, change or remove a navigator.
